Calcium is a mineral that keeps your bones, muscles, nerves and heart working properly. This test measures the calcium in your blood. A high level may relate to overactive parathyroid glands, certain medications or other conditions and can cause tiredness, thirst or kidney stones. A low level can cause muscle cramps, tingling and, if severe, heart rhythm changes. The result helps your doctor check bone health and how your body balances minerals.

You do not usually need to fast for a calcium test, so you may come at a time that suits you. If your doctor has ordered other tests at the same time, fasting may still be required. Tell the nurse about any calcium or vitamin D supplements, antacids, and all medicines you take. Do not stop prescribed medicines or supplements unless your doctor or the laboratory tells you to do so.
A large portion of calcium in the blood is attached to a protein called albumin. If albumin is low, the total calcium result may appear low even when the biologically active, or ionized, calcium is normal. For this reason, doctors often interpret calcium alongside albumin, use an albumin-adjusted calcium result, or occasionally request an ionized calcium test before reaching a conclusion.
Not necessarily. The body keeps blood calcium within a narrow range, and may draw calcium from the bones when needed. A normal calcium result does not, on its own, rule out vitamin D deficiency or assess overall bone health.
A result that is only slightly outside the reference range is common and may be repeated before any decision is made. Your doctor will consider it alongside albumin, kidney function, vitamin D, PTH where appropriate, your symptoms, and the medicines or supplements you take.
